细胞核NR4A3免疫染色是涎腺腺泡细胞癌特异和敏感的新标记物
      摘要:最近,我们发现[t(4;9)(q13;q31)]重复性基因组重排通过增强子劫持使转录因子核受体亚家族 4,组 A,成员 3(Nuclear Receptor Subfamily 4 Group A Member3,NR4A3)上调是涎腺腺泡细胞癌(acinic cell carcinoma,AciCC)的致瘤驱动事件。本研究对共计64例包含17%高级别转化的AciCC、29例分泌性(乳腺样)癌[secretory(mammaryanalog) carcinoma,MASC]和70例其他涎腺癌进行对比,评估了NR4A3免疫染色和NR4A3荧光原位杂交(fluorescence in situ hybridization,FISH) 在AciCC鉴别诊断中的作用。细胞核NR4A3免疫染色是AciCC高度特异(100%)和高度敏感(98%)的标记物,仅1例为阴性,而NR4A3 FISH敏感性相对较低(84%)。MASC和其他涎腺癌均未显示出任何细胞核NR4A3免疫染色。在49例可评判的AciCC中,有4例(8%)存在最近描述的HTN3-MSANTD3基因融合,所有4例均有细胞核NR4A3免疫染色。综上,NR4A3免疫染色是AciCC高度特异和敏感的标记物,可能在高级别转化的病例以及AciCC和MASC的鉴别诊断中对缺乏“酶原颗粒”病例的诊断尤其有用。
